With a sad heart, we are announcing that we are permanently closing our clinic doors. It has been a true privilege to serve you and this community.
In the interim, those with urgent medical needs please go to Urgent Care or the Emergency Room. Those that have found new primary care providers please contact their office or use the Urgent Care or Emergency room.
All medical requests relayed to our office (if you spoke to the nurse or left a voicemail) prior to September 8th, and medical professionals seeking coordination of care requests will be addressed.
Unfortunately, no new requests can be attended to. If you haven’t already, we urge you to establish care with a new primary care provider. We will continue to monitor messages through September 30th.
Refills:
All refills or prescription renewal requests MUST come from your pharmacy – NO EXEPTIONS. Please contact your pharmacy, and they will send your prescription request directly to Dr. Seleverstov. No refills can be processed by calling the clinic.
Medical Records Requests:
Your medical records should be available through the “MyChart” patient portal system if you have signed up for an account (typically if you have been to Providence or Kadlec you may have already signed up for MyChart).
If you desire a hard copy of your records, please fill out a Release of Information (ROI) form. You can email it to office@seleverstovmedical.com. A standard medical records processing/materials fee will apply (see ROI form for details, form will be available by 9/14/2026 for download).
Lab Results, Imaging Reports, etc:
Depending on which lab you went to, your lab results are available through the patient portal systems of Labcorp or MyChart (Providence on Chase Street). X-ray, CT, MRI reports can similarly be obtained at the facility you went to for the scans (Providence, Walla Walla Clinic, Dayton Hospital, etc).
Billing:
Account balance statements will be mailed out soon.
